  • Multiple medications requiring instructions/monitoring

  • Ongoing management of indwelling catheters

  • New diagnosis requiring instruction

  • Frequent falls

Respite Care (Relief for caregivers)

Personal Care Assistance (Bathing, dressing, personal care and grooming)

Companionship/Sitter (Reminisce, correspondence, organize mail)

Medication Reminders (Management, observe and monitor)

Meal Preparation (Special attention to nutritional needs.  Plan, prepare and clean up)

Homemaking (Laundry, change bed linens, clean bathrooms and living areas)

 Transportation & Errands (Church, beauty shop, doctor visits and shopping)

Household Organization (Clear out clutter, sort paperwork and provide organizational solutions)

Medicare eligibility includes:

1. The patient must be a Medicare beneficiary.

2. The patient must have a need for skilled services.

3. The patient must require considerable and taxing effort to leave home or temporarily or medically restricted.

4. The patient must meet the criteria for part-time or intermittent services.

5. The patient must be under the care of a physician (MD, DO or Podiatrist).
